What is Enhanced Care Management?
Enhanced Care Management (ECM) is an innovative care model that offers personalized, in-depth management of healthcare and social needs. It’s a coordinated approach that addresses the full spectrum of an individual’s health, including medical, behavioral, social, and practical aspects of their daily living. Our ECM services are particularly beneficial for those with multiple chronic conditions or complex healthcare needs.
Our ECM program aims to improve health outcomes, enhance patient satisfaction, and reduce overall healthcare costs. By addressing healthcare needs comprehensively, ECM at Heritage Health Network ensures more coordinated, efficient, and effective care delivery.
ECM is designed for individuals who require more intensive healthcare management, including those with chronic diseases, high healthcare utilization, or multiple healthcare needs To be eligible for ECM, members must be enrolled in a Medi-Cal managed care health plan and meet criteria for at least one of the Populations of Focus.
DHCS has identified both adult and children/youth Populations of Focus for ECM..
