Our populations of focus

Our members are never defined by labels, but we focus on serving individuals facing the following life circumstances to provide the right support.

Individuals experiencing homelessness

Individuals at risk for avoidable hospital or emergency department utilization

Individuals with serious mental illness (SMI) or substance use disorder (SUD)

Individuals with complex health and social needs (including those with multiple chronic conditions)

Individuals who are pregnant and experiencing high risk

Children or youth with serious emotional disturbance (SED), SUD, or those involved in child welfare

Individuals at risk of institutionalization or eligible for long-term care

Adults and youth with limb loss or at risk of limb loss

Adults and Youth with diabetes

Justice-impacted adults: Adults that have current or past involvement with the criminal justice system, including incarceration, probation, or reentry.

Justice-impacted youth: Children and adolescents who have current or past involvement with the juvenile justice system, including detention, probation, or reentry into the community.
