How We Help Students & Families

Our ECM team partners with educators, parents, and community-based organizations to ensure each child has the care and resources they need to thrive. Our Lead Care Managers will:

  • Coordinate Health Care: Link children with medical, dental, and behavioral health providers.
  • Support Emotional Well-Being: Offer counseling and behavioral interventions that build resilience.
  • Create Individualized Care Plans: Collaborate with families and educators to tailor support to each child.
  • Connect Families to Resources: Guide parents toward food, housing, and financial supports.

How It Works: Step-by-Step

Onboarding

Receive your Partnership Onboarding Package, review program details with our team, and complete the forms required to begin participation.

Identify Students In Need

Once your team identifies students who can benefit from the ECM program, HHN will facilitate enrollment and authorization of services.

HHN Engages Students & Family

Our team contacts students and families by phone, in person, or in the office to begin care coordination and ensure they receive timely support.
