Mary’s* story

Where Our Member Started 

When Heritage first met Mary*, she had just lost her vision due to poorly managed chronic conditions. She was scheduled for surgery in November and was very much looking forward to it, especially the joy of seeing her grandchildren open their Christmas gifts and simply being able to see the Christmas tree.

 

Unfortunately, her surgery was postponed due to her A1C levels being above 12.2, and subsequently close to 13.

How Heritage Helped 

Mary began working tirelessly with her to close care gaps with her PCP, meeting with them on a monthly basis to get her condition under control. Finally – her A1C come down to 8, and she was cleared for surgery on Monday June 9,2025 at 11:20 AM in Upland, CA.

Member Outcome

The surgery was the start of a new life – one where she could clearly see her grandson! She was overjoyed with the news, so overcome with emotion that she cried on the phone, as she had begun to lose hope of ever regaining her vision. 

 

I will be providing her transportation to the surgery and will also try to be there to support her if she attends alone. Today, Mary remains in our program, has better controlled AC1 see levels and can see her family and a happy future ahead!
