Moses's Story

Where Our Member Started 

When Moses became a Member of Heritage, he was a 7 year old that was missing a great deal of school due to lack of transportation, and unstable housing. 

 

He and his family where staying in motels with his mom and sibling but had no consistent place to call home. This young Member was having a lot of trouble with Behavioral Health issues in school and at home. Sadly his  PSC-35 was 35 at the time of joining the Heritage Program.

How Heritage Helped 

Jenne got to work right away –  submitting BH Referrals, ensuring the Member received ABA speech therapy, and group counseling in school. At the same time, on a parallel track, she worked with the Member’s mom to attain a new housing referral, and find a stable home for them.

Member Outcome

Today our young Member is THRIVING. He has performed so well in school he recently received the “Student of the Week” award in October and November of 2024! He so proud as he received a Student of the Month Award in front of his whole class on December 3, 2024. Since the beginning of the school year the patient has had only 3 excused absences.

 

This young Member now has stable housing as the family moved into their new apartment on November 1 2024. With donations from LCM and Heritage the families even has furniture for their new place as well! In early 2025, the LCM was able to retest the young Member’s PSC-35 – and it dropped down to 23, quantifying the big impact all of the positive change has had.
