Diabetes & Limb Loss Preservation

HHN delivers whole-person care for members with uncontrolled diabetes, chronic wounds, and limb loss. We coordinate medical care, behavioral health, and social supports to stabilize health, prevent complications, and protect mobility. Our team connects high-risk members to wound and vascular care, prosthetics, rehabilitation, and mental health support to address the emotional impact of limb loss and chronic disease. This integrated approach improves quality of life and drives better limb preservation and long-term outcomes.

Unmanaged Diabetes

Members with a confirmed diagnosis of diabetes (Type 1 or Type 2) who show signs of poor glycemic control or lack of disease management.

Diabetes with Risk for Amputation

Members with diabetes who exhibit complications that increase risk of limb loss but have not yet had an amputation.

Diabetes with Amputation

Members with diabetes who have undergone a lower extremity amputation (toe, foot, or limb).

Amputation with Risk for another Amputation

Members with one or more previous amputations who show clinical indicators for potential subsequent amputation.
