What is Enhanced Care Management (ECM)?

It’s a Medi-Cal program in California that provides extra support for people with complex health and life challenges.

 

Our team acts as an extension of a Member’s care team—trusted advisors who help navigate healthcare and social services. We provide a personal care team to keep Members healthier and connected to what they need, from medical and mental health care to housing support or safe transitions home from the hospital.

Instead of leaving individuals to figure it all out on their own, ECM connects all the dots between doctors, specialists, social workers, and community programs. The goal is to make life easier, healthier, and more stable, especially for people who face serious or ongoing challenges like homelessness, chronic illness, mental health issues, or involvement with the foster care or justice system.

What ECM Provides

Continuous monitoring, assessment & follow-up

Ongoing health monitoring and reassessment, ensuring timely interventions and continuous support for health improvement.

Crisis intervention & emergency planning

Rapid response in crisis situations, with effective emergency planning to manage and mitigate health risks.

Healh education & blood pressure management

Educating members on health practices, specifically focusing on managing and controlling blood pressure effectively.

Comprehensive transitional care

Ensuring smooth transitions across different care settings, with a focus on maintaining continuity and quality of care.

Emotional support & empowerment

Providing psychological support and empowering individuals to participate in their healthcare decisions and management.

Addressing SDOH & coordinate community resources

Identifying and addressing social determinants of health and coordinating with community resources for care support.

Comprehensive assessment & care plan

Thorough evaluation of individual health needs, leading to a personalized and dynamic care plan for each member.

Otutreach & engagement

Proactive identification and connection with eligible individuals, fostering relationships and engagement.
