HHN Lead Care Academy: Excellence in Enhanced Care Management
The HHN Lead Care Academy is our branded training platform designed to set the gold standard in Enhanced Care Management across California. Every care manager who joins Heritage Health Network is supported by a structured, role-specific learning pathway built around CalAIM’s seven core ECM services and payer requirements.
Through the Academy, Lead Care Managers, RN Care Managers, Behavioral Health Care Managers, and LCSWs are equipped to deliver care that is consistent, audit-ready, and outcomes-driven. Training covers everything from effective member engagement and care planning to documentation practices and compliance essentials—ensuring quality performance that drives measurable health improvements.
