Our populations of focus
At Heritage, we never lose sight of the fact that each member of the population is a person. We know individuals are not defined by their circumstances, and our goal is to help each person meet their individual potential.
Individuals experiencing homelessness
Individuals at risk for avoidable hospital or emergency department utilization
Individuals with serious mental illness (SMI) or substance use disorder (SUD)
Individuals with complex health and social needs (including those with multiple chronic conditions)
Individuals who are pregnant and experiencing high risk
Children or youth with serious emotional disturbance (SED), SUD, or those involved in child welfare
Individuals at risk of institutionalization or eligible for long-term care
Adults and youth with limb loss or at risk of limb loss
Adults and Youth with diabetes

Medi-Cal members are eligible for the ECM Benefit if they meet the ECM Populations of Focus eligibility criteria as defined in this checklist and are not enrolled in duplicative services (as defined in the Exclusionary Checklist)
